Annual membership fees

Annual Dues are based on patients age.

Couples rate - discounted $300 from total cost

Each additional family member beyond two - discounted $500

Payment Plan

Annual payment at time of enrollment and yearly by check. Monthly automatic credit card billing with 5% service charge.

This practice provides only the limited scope of primary care services specified in the retainer medical agreement. Additional services not specified in the retainer medical agreement will be billed separately.
